To identify a conjugate acid-base pair: 1. Identify the acid on the right. 2. The conjugate base of this acid will be this acids without a 4 $H^+$, so search it on the other side of the reaction. 3. Do 1 and 2 for the base on the right, but looking for the base with a $H^+$ in it, you will find its conjugate acid.
