Where did cole swim to in chapter 5

Cole has the idea of swimming from island to island and eventually to civilization. Cole returns from a flashback of the healing circle back to the frigid water. He is having trouble simply swimming to the next island. His leg is cramping and the tide is pushing him back to shore.
