The most obvious mistake is the fact that he makes the trip alone.

A second mistake occurs when the ice breaks and he soaks himself in the creek, leaving him exposed and putting him further behind in his journey.

A third mistake can be found in the location he chooses to build his second fire, as he fails to consider the snow in the tree above him. Panicked, he uses all of his matches and is left without fire, as well as the ability to make another.

As for which is most responsbile for his death... I'll allow you to choose.
