What is an internal conflict in Chapter 1 in The Pearl?

What is an internal conflict that happens to Kino in Chapter 1? It does not have to be directly towards Kino, it can be a conflict on another main character.

Kino does not want to take his baby to see the doctor because the doctor looks down on Kino and his people. Kino must reconcile with this because he is desperate to get Coyotito medical attention.
