Johnny changes his perspective regarding his mother. Earlier he said " i stay away all night, and nobody notices. " now she notices but he refuses to see her.

What might be a cause for this change?

Do you mean when Johnny is in the hospital? Johnny refuses to see his mother. He is convinced that his mother does not care about him so why should she care now? Pony's family is closer to Johnny than his mother.
