And why didn't all the Career Tributes make it through the first day? Who killed the district 4 male tribute? And why? Was it because of his small size?

After a few glimpses District 4 boy trying to escape, he runs out from the Cornucopia to meet Cato. It is unknown how exactly he died, but it is likely his throat was slit by Cato's sword.
