Offred is mandated to visit the doctor's office once a month because she is fertile. The Guardian accompanies her to these appointments where she is tested for pregnancy and disease. The doctor tells her that most Commanders are too old or sterile to produce babies. The doctor offers to impregnate Offred himself. Handmaidens are merely faceless vessels to produce offspring.
