why could Uncle Taher not get the help he needed?(Passport)

Uncle Taher needed a passport to leave and travel to England for surgery. Unfortunately, the director of the hospital (a former window washer), says that he will receive the passport if God wills it......... which we can infer means he isn't going to help.
