McMurphy is metaphorically smothered by white uniforms the day he comes in. THe Doctors and Nurses (Ratched in particular) try to drown him in a sea of institutionalized nonsense. He fights this regimented world of forced conformity to his last breath until he is literally smothered by a white uniform.
