why does the author choose to have McMurphy "smothered"in the white uniforms?

McMurphy is metaphorically smothered by white uniforms the day he comes in. THe Doctors and Nurses (Ratched in particular) try to drown him in a sea of institutionalized nonsense. He fights this regimented world of forced conformity to his last breath until he is literally smothered by a white uniform.
