Why did Johnny imply to Doctor Warren that his hand had been crippled since birth?

Dr. Warren notices Johnny’s deformed hand and offers to examine it. However, Johnny lies and says the injury was a birth defect and he doesn’t need any help. Jonny is self-conscious about how he actually injured his hand. He does not want to recount the story to Dr. Warren.
