He measures Marlow's skull as part of a scientific experiment. The doctor feels that in Africa, "the changes happen inside." He thinks that white Europeans have no business in the darkness of Africa and, if they come back, are changed. He thinks that Marlow's physical head shape might actually be different when he returns.
