Volume Four of the text is an excercise in digression. Important points are actually hard to discern. For the most part, important parts of the discussion include the importance of names, and the importance of having real meaning in life..... things you truly care about. Note, each of these topics revolves around Tristram's father's philosophical discussion about his nose.... or shall we say the size of his nose.
