why is mr.dussel being described as “A quack at work” ?

Mr. Dussel pulling out Mrs. Van D's tooth out was a comedy of errors. She flailed around while people laughed at the scene. Mr. Dussel did not do too much other than yank the tooth out while she moved all over the place.
