What happened to rachel?

In chapter 11 one learns of Rachael’s death after Hannah begins to question Esther about her. The girls have come off the trains and been ordered to remove their clothing and prepare for the showers. Hannah recalls that Rachael always had trouble breathing evidenced by her peculiar breathing patterns.
