Part of Chief’s madness is seeing things that don’t really exist. Yet on a more elemental thoughtful manner, they do exist. This is what makes Chief such a brilliant character. He sees colours like "green slime" oozing from the pores of the doctors that speak so knowingly about the men. He sees the "frozen" light rods of a very cold institution. Chief aware of his environment on a whole different level than the others.

Are you talking about the fact that the Chief always believes himself to be in a fog, but when he alone (such as page 130) in the hallway............ everything is clear and unmarred. If so, the imagery would be Chief's feeling lost in the fog.
