based on the opening scenes of the movie, why did so many soldiers lose their lives during the Civil War?

That was a brutal battle scene. Basically, when they got injured, the conditions were not sanitary. Infection set in immediately. The doctors could not hack limbs off fast enough. If you didn't die from the injury, you probably died from the medical treatment. THe method of warfare, charge and shoot, was pretty much suicide as well.
