What decision in Steves past does he regret?

I don't think it is explicitly stated (I only saw the movie) but at the end he breaks down and weeps. He wishes he had acted differently with regard to Miss Kenton and Lord Darlington. Both people were close to him, for different reasons, and he just couldn't bring himself to express his emotion.
