Why does the doctor pay kino a house call

The doctor has come to check on Coyotitio, who he'd already turned away as a patient because Juana and Kino had no money. New of the pearl's discovery changes all that.... and the doctor is miraculously one of the first to visit. The doctor is not there for the child.... he's there for the money the pearl might afford him.
