What is the doctor's reaction to Kino at the gate? What does Kino try to pay him with?

The doctor is European. He hates Kino and his people. The doctor sees them as poor savages. He wants to know if they have anything to pay before he sees them. All Kino has is a few tiny misshapen pearls....worthless.
