Kino begins to beat at the gate in rage as soon as he arrives. The gate symbolizes the divide between Kino and the doctor..... the divide between who the doctor will treat, and who he will not. Kino gives into his fears before faced with them, all while holding his hat in his hand as a sign of respect and submission.
