Best Answer: The beggars no the doctor is useless to them. THey know he only cares about money and will not treat people who don't have it. THe Doctor would rather be relaxing in Paris. He knows he is greedy and uses his title to take advantage of people. Kino is desperate for his baby. When the doctor initially refuses to treat the baby, Kino is humiliated and angry with the doctor.
