There is conflict throughout the story, As soon as Kino finds the pearl, Kino has fractured his relationship with friends and extended family. There is an "otherness" to Kino now that he will be a wealthy man. The peaceful status-quo has been altered. Some are jealous and some are envious of Kino.

The doctor despises Kino and his people. He does not want to help Kino's baby until he hears that Kino has found the "pearl of the world". After that, he makes Kino's baby sick so Kino thinks the doctor is of value. The doctor repeatedly tries to take the pearl from Kino.
