Rachel was Tom's mistress. Toward the end of the novel, we also learn that he was having an affair with Megan.

Rachel was Tom's ex-wife. When Tom was married to Rachel, Anna was his mistress. Rachel later found out, causing her and Tom to divorce. Tom later married Anna, and then his mistress was Megan, whom he later killed.
