Liesel usually sees her brother coughing up blood and dying on the train in her nightmare. Now she sees Max doing this in her nightmare.

how does it represent what she is facing in her own life?

Liesel is constantly worried about losing Max, a brother -figure in her life. Max is sick and her fear manifests itself in her dreams.
