Sherlock Holmes did not want Watson to examine him. This is first of all because of Dr Watson who as a doctor could easily catch the truth that Holmes was pretending to be ill. Secondly, it was utmost important for Holmes to convince Watson the severity of the disease and Watson too was not at all good at hiding his feelings in front of Culverton Smith. If Holmes would have shared the secret with Watson, he would never have been able to impress Smith with the urgent necessity of his prescence to treat him.

Holmes told Watson that the disease with which he was bed ridden was the deadliest Collie disease from Sumatra and it was most horribly contagious. He also told that Watson knew nothing about these eastern diseases. This is the reason he gave to Mr. Watson.

He asked Watson to bring Mr. Culverton Smith of 13, Lower Burke Street who was a planter not a doctor and was involved in the murder of Victor Savage, his nephew.
