In a normal term pregnancy, the amniotic sac may rupture in th middle of the dilation stage of labor. Dilation, typically, may lasts from 19 to 18 hours--or more in primigravidas. Therefore, even if the fluid leaking from the patient is amniotic fluid-- not urine or a mucous secretion-- there is enough time to get the patient to the hospital.

The husband's claim that he can see the baby's, head confuses the issue. Normally, the baby's head becomes visible in the expulsion stage. Before this phase, lightening ( baby's descent into true pelvis) and effacement would have occurred; also the cervix would be have fully dilated. Crowning occurs in middle or late expulsion. If the mother is a multigravida, and she has been bearing down during the episodes of contraction, the end (birth) could come very quickly. Assuming the husband is not mistaken, then an emergency medical ambulance with a doctor or a midwife aboard should be summoned.
