how would be different if this story had been told by the murder victim, speaking from the afterlife?

on the last paragraph of The Tell-Tale Heart

The last paragraph has the narrator going loony-tunes. If you mean a shift in point of view in the last paragraph, this would have drove him extra crazy (if that's possible). If it had been the victim's POV from the start, I think he would have been largely clueless of the narrators intents.
