McMurphy finds out from a patient lifeguard that someone who is committed to the hospital can only be released at the discretion of the staff doctors and nurses. McMurphy had thought he could leave as soon as he served the time. The pool is symbolic of McMurphy drowning in a situation he can't swim in.
