What is some actual source of Chief Bromden's fog?

In Chapter 15, we learn that insanity is the source of the fog, and the most important point that Chief Bromden makes in these chapters is that the “insanity”, which is represented by the fog, is a comfort for the patients. The fog (insanity) allows them to recede from the difficulties of reality.
