I don't think it is the trip that exhausts McMurphy. I think it was his increased responsibility for the mental health of the men that he feels. As well his issues of being dominated by a woman wear on him. From his mother to Nurse Ratched, McMurphy is tired and insecure (note the life jacket as well!)
