The only thing I can think is that the people in this novel think that if you are not one way, you are the exact opposite.  They do not see any middle ground.  Slim looks at Lulu's pups and sees two types: healthy and runty.  Small=useless.
Curly looks at George and Lennie's friendship and sees it as "that way" because he cannot see the possibility of two men being true friends; men are loners who look after themselves.  Even Lennie sees only good and bad.  He just isn't intelligent enough to see middle ground.  Carlson and the others see Lennie as a criminal and want him dead.  They cannot see the possibility that he cannot help the way he is and deserves different treatment.
