Gertrude drinks from the poisoned goblet.

Claudius is killed when Hamlet wounds him with a poisoned sword, after which, Hamlet forces him to drink from the poisoned goblet.

Hamlet also dies from a wound inflicted by the poisoned sword.

Laertes also dies from a wound inflicted by the poisoned sword after wounding Hamlet.
