Why doesn’t Warren have a psychiatrist? What is Keyes saying about psychiatrists and/or funding for such institutions?

Charlie’s visit to Warren ends badly, with Mr. Winslow expressing how there are not many people willing to do the hard work that the psychiatric facility requires. People simply don't want to do the job. I think the author is implying that these institutions are underfunded.
