Dunstan had been in another place, in his head, before he awoke at the hospital. He had been in another peaceful realm, an altered state and had drunk the milk of paradise. The allusion to Coleridge's Kubla Kahn is interesting. Dunstan preferred the imaginary state to his reality. In this state he could reply to voices or not. There was a liberation in it.
